The contents of this form will be accessed by our staff at the Authority and our web provider Mando Group, who will respond to the application. Information provided during applications will be stored on a database.

MWDA reserves the right to use this information and any details recorded by staff to review website usage and to prevent misuse. This information will not be passed on to any other party without your consent. Information will only be used to monitor website usage and deliver MWDA services.

As a public authority, MWDA complies fully with the Data Protection Act, Freedom of Information (FOI) Act and Environmental Information Regulations (EIR).
